Added login_log table to track login attempts.
Added login_failure table to track failed login count.
Renamed sessionlog table to session_log.
Added login_log_id to session_log table.
New include file added for login tracking and blocking, as includes/login-functions.inc.php
Adjusted the login.in, session.inc and logout php pages to accomodate the new login blocking and tracking.
Fixed some LDAP testing bugs.
